Default EWP malfunction leads to death of tree worker

On Thursday, a 45-year-old tree feller died in Lara after his cherry picker malfunctioned, throwing him 15m to the ground.

It is believed the machinery had been malfunctioning earlier in the day.

That article has 2 different incidents in it. My Ops Manager had the article yesterday to show us. A tree crew in Lara, last tree out of 20 trees being pruned. The operator was thrown out of the bucket after the tower suffered a mechanical error, he was thrown several metres to his death. Don't know what sort of tower, don't know if he was wearing his harness or if it was attached. Going to have to wait for the report to come out.

From that photo I wonder if the Ewp is their own machine or a hire one???

I don't wish to be disrespectful but malfunctioned how? I'm sure a lot of us own or have used a similar machine and would appreciate any info on what to be looking out for.

I'm curious as well. Most people use towers and could encounter a similar problem. It sounds/looks like the hydraulics failed in the top boom dropping it a bit.

Looks like a trailer mounted snorkel ewp.
