Default sick willow tree

I have a 10 tear old weeping willow in my front yard. We planted it there without thinking that it might not be the best spot for it. Its about 25 ft from the front of the house and unfortunatly very close to our water lines and well. I tell you this only because this information might help you when answering my question.
This tree srarted out well in that it grew fast and was very full. But in the last year it seems to have gotten thin and has alot of dead limbs with no leaves. It also started losing leaves in the last couple of months. They turn yellow and fall off. Not just a few, ALOT. Plus I noticed a big hole where the ground meets the trunk. Almost like the trunk is rotting. My question is what is wrong with my willow and can it be saved?
