Exclamation Help with my Nuttall Oak....

Good evening, I am hoping that someone here can help me with my Nuttall Oak. About a month ago, I noticed a section of bark that came off the trunk section of my oak tree. Some of the surrounding bark is loose as well, but still attached. Today I noticed a branch with brown leaves. Upon closer inspection, I noticed a dark area where the branch meets the tree (see photos). I don't know if this is a insect/borer problem? Also notice the small (1/4 inch or so) hole (see photos). What should I do??? I DO NOT WANT TO LOOSE THIS TREE!!!! I live in south west Kansas and we dont have very many trees here. please advise on what I should do....Thanks

Hard to be sure from the pics, but looks like the tree is planted too deep. The part with bark missing may be mechanical injury, but there appears to be bark cracking above it, A pic of the base of the tree along with the ground level may give us some ideas.

If too deep there are remedial things to do. How long has the tree been here? How was it planted - tree spade? bareroot? ball and burlap? How much water does it get? What kind of soil is it growing in?

The tree was balled and burlap when I planted. It has been here 2 1/2 years.

Also there is some bark cracking on the other areas of the tree as well.....

was the burlap removed? was the root collar located at ground level?
