Thick (3-5") of mulch should smother grass and help build callus around that wound.

It's a young tree with a bad start of it's mature live. Get the growing conditions a good as possible and the wounds will easily be overgrown and no big problems in the future. Get rid of everything underneath the dripline. Clean it up till you get to the roots, then apply a thin layer of composted mulch. Then sit back and let the tree do it's thing.

Should I ensure that the mulch doesn't cover the wound?

Yeah, otherwise it might hold moisture in the wound and cause further decay.

I saw someone using a short piece of 4-inch weeping tile hose at the base of their trees. It looked like a good way of preventing lawn mower damage, and keeping mulch away while still allowing air to circulate?

The mulch should keep grass away and make easy enough to pull up any weeds/grass. May be faster and more effective than weed whipping too.
